I second the Pivotal Tracker recommendation....yes the fact that I worked on it extensively while I was at Pivotal Labs (legal and otherwise) makes me biased, but I know SO many startups and established companies that were thrilled beyond belief with it...you can actually go to Amy's company's website (plug for Get Satisfaction here) and the customer service stream:
